Faze Contest: Girlz' Night Out

The “Girlz’ Night Out” contest (the “Contest”) is sponsored by Faze Publications Inc. (the “Sponsor”).

ELIGIBLITY: Contest is open to female legal residents of Canada between the ages of 13 and 20 excluding residents of Quebec, employees of the Sponsor, their parent and affiliated companies, representatives, dealers and agents, contest suppliers and judges, and those with whom they are domiciled. Entrants under the age of 18 must have her parent or legal guardian’s permission to participate in the Contest. By entering the contest, each participant acknowledges having read these rules and regulations.
The Contest Period starts at 12:00 AM on April 1, 2009 and closes at 11:59:59 PM on May 18th, 2009 (the “Contest Period”). All times are Eastern Times. NO PURCHASE NECESSARY.
HOW TO PARTICIPATE IN THE CONTEST: To enter, go to www.faze.ca and click on the contest link and select “Win a Girlz’ Night Out”. Complete all mandatory fields in the online entry form, including name, address, age, city, postal code, phone number, and email address.
Draw: A random draw by a representative of the Sponsor from all entries received will be made at or around 9:00 AM on May 19th, 2009 at the office of the Sponsor. The selected entrant will be contacted by telephone, email, or courier. In the event the selected entrant cannot be contacted within two (2) days of the first attempt of contact, another selected entrant will be chosen.

Prizes must be accepted as awarded without substitution and is not transferable, not for resale and has no cash surrender value. The Sponsor reserves the right, in the event that a Prize or any component of the Prize cannot be awarded as described for any reason, to substitute another prize or component of a prize of equal or greater value, without liability. The winner and his/her guest must depart from and return to the same gateway.
In the event of a dispute regarding who submitted an on-line entry, the entry will be deemed submitted by the holder of the e-mail account. The sole determinant of time for the purposes of receipt of a valid entry in this contest will be the contest server machine(s).

PRIZE: There will be one (1) Grand Prize awarded, consisting of one (1) trip for two (2) to Toronto, Ontario to participate in the Girlz’ Night Out. The Grand Prize is more specifically described as follows:

Return transportation for two (2) to Toronto, Ontario as outlined;

- Two (2) return economy airfare tickets from the major airport closest to the winner’s Canadian residence to Pearson International Airport, Toronto, ON

Two (2) nights accommodation
All meals for the winner and guest for the duration of the trip

Approximate retail value of the Grand Prize is $3,000. Actual prize value depends on location of winner’s residence and flight schedule. No financial compensation will be made or required if actual costs are lower or higher than this amount.

THE WINNER

It is the winner's and his/her guest's responsibility to provide for any additional expenses not included in the Grand Prize as described herein, including, but not limited to, travel insurance, gratuities, return transportation between the winner's home and the major airport nearest his/her Canadian residence.
The winner and her guest must travel together on the same itinerary. Trip must be booked and taken on the dates and times specified by Faze Publications Inc.or its agent, or will be forfeited, in Sponsor’s discretion. No responsibility is assumed by Sponsor for canceled, delayed, suspended or re-scheduled events or travel after ticketing for any reason whatsoever beyond its control. Transportation and accommodations are subject to availability, blackout periods, government restrictions and regulations, hotel, airline, airport or other transportation restrictions and regulations. Other restrictions may apply.
Each potential winner, or her parent or legal guardian, will be required to sign a release form consenting to the use of all or part of photos and videos for promotion, or publication in Faze Magazine and/or www.faze.ca without any further consent or remuneration.

GENERAL: By entering the Contest, entrants agree to abide by the Official Rules and the decisions of the Sponsor’s organization with respect to all aspects of the Contest. The decisions and rulings by the Sponsor is final and without appeal in all matters related to the awarding of the prizes. Odds of winning depend on the number of entries received.
To be declared a winner, the potential winner must first correctly answer, unaided, without assistance of any kind, a mathematical skill-testing question. Each potential winner, and/or her parent or legal guardian will also be required to sign a declaration and release form (the “Release Form”) confirming compliance with the Official Rules, acceptance of the Prize as awarded, without substitution, and releasing the Sponsor, their affiliated and related companies, advertising and promotional agencies, and their respective shareholders, directors, officers, employees, agents, representatives, successors and assigns from any liability in connection with the Prize or this Contest. The duly completed Release Forms must be returned within the time period specified on the Release Forms or the Prize will be forfeited. A selected entrant who is not at least of the age of majority will require her parent or legal guardian to act on her behalf in regards to accepting the prize. The delivery of the Prize will be arranged through the Sponsor within thirty (30) days of receipt of the duly completed Release Form. Winner’s guest must also sign and return a release prior to being ticketed or within such other period as Sponsor may specify. Trip winner’s guest cannot be changed after the release is received.If the winner is a minor, the minor’s travel companion must be a parent or legal guardian of the minor, or such parent or legal guardian must provide the Sponsor with a release and consent to another adult traveling with and taking responsibility for the minor.
The Sponsor accepts no responsibility for loss, damage or claims caused by or resulting from the Contest or any Prize associated with this Contest. By entering and accepting the Grand Prize, the winner and her guest consents to the use of their name, address (city and province) and/or photograph/video image without further remuneration, in connection with any publicity carried out by or on behalf of the Sponsor with respect to this Contest.
The Sponsor will not be responsible for late, lost, illegible, incomplete or destroyed entries and all such entries are void. The Sponsor and the independent contest organization do not assume any responsibility for incorrect or inaccurate capture of entry information, technical malfunctions, human or technical error, printing errors, lost entries, or any combination thereof. Entries submitted through illicit means or spamming will be disqualified and any entry materials that have been tampered with or altered are void. In the event it is determined that an entrant has entered in a fashion not sanctioned by these rules and/or has submitted more than the number of entries permitted by these rules, the entrant will be disqualified and all of the entries submitted by the entrant will be disqualified.
The Sponsor reserves the right to terminate or amend this Contest at any time and in any way, without prior notice. Without limiting the foregoing, if, for any reason, the Contest is not capable of running as originally planned, such as tampering or infection by computer virus, the Sponsor reserves the right to cancel the Contest and conduct the draw from all previously received eligible entries received by the Contest Closing Date. The Sponsor shall not be held responsible for any problems, errors or negligence that may arise or occur in connection with the Contest including but not limited, to any damage to an entrant’s computer equipment, system, software or any combination thereof, as a result of the entrant’s participation in this Contest. Any attempt to deliberately damage any web site or to undermine the legitimate operation of this promotion is a violation of criminal and civil laws. Should such an attempt be made, Sponsor reserves the right to seek remedies and damages to the fullest extent permitted by law, including criminal prosecution.
